- Maximum order: $1,000,000 per order. Minimum order: Travel Assignments are 13 weeks at 36-40 hours per week. Supplemental / Per Diem assignments are 4 hours. Supplemental/Per Diem cancellation must be made within 2 hours prior to the start of a shift or a charge of 2 hours will be made. 621-025 Registered Nurse - General, Medical Surgical Education: Graduate of accredited school of nursing and current license to practice in the state in which employed. Certification: Candidate must have Basic Life Support (BLS) Experience: Minimum of 1 year experience. Licensure: Valid license in at least one state.
621-025 Registered Nurse - Operating Room, ICU, ER, OB/GYN, Coronary Care, Pediatric Education: Minimum education would be an associates or bachelor's degree in an RN Program from a National League of Nurses (NLN) accredited institution. Certification: Candidate must have Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) Experience: Minimum of 1 year experience. Licensure: Valid license in at least one state.
621- 038 Licensed Practical/Vocational Nurse Education: Graduate of accredited school for LVN/LPN Experience: 12 Months of accumulated experience within the past 5 years; new graduate in lieu of twelve months of accumulated experience, recently graduated employees persons will be eligible for employment after completion of 1200 verified hours of employment with in the past twelve months. Certification: Candidate must have Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) Licensure: Current license to practice in the state in which employed.
621-040 Certified Nurse Assistant Education: Completion of a basic aide training program consistent with state and federal requirements and for which a certificate was obtained. Experience: Completion of 1 year employment as a nurse's aide in a hospital or health-related facility. Certification: Must have Basic Life Support (BLS) training.
Medical Liability Insurance Limits: $1,000,000.00 per occurrence; $3,000,000.00 aggregate. For services, the ordering procedures, information on Blanket Purchase Agreements (BPA's), are found in the Federal Acquisition Regulation (FAR) 8.405-3.
